本業務は、事業委託者である東京都の事業として実施しており、必要に応じて業務の一部を第三者に委託する場合があります。 その場合には、十分な個人情報保護水準を備えた委託先を選定し、契約等により適切な管理・監督を行います。

（5） 入力内容について
入力フォームの必須項目には必ずご入力ください。 必須項目にご入力いただけない場合は、お申込みを受け付けできない場合があります。 また、入力内容に不備がある場合は、事務局より確認のご連絡を差し上げることがあります。 任意項目については入力は任意ですが、円滑な運営のため可能な限りご協力をお願いいたします。
